COVID-19

  • Do you have any of the following symptoms: a cough, or a fever?

  • Have you been in contact with anyone that has tested positive for COVID-19 or any other communicable diseases?  

  • Have you travelled outside the province in the last 14 days?

  • What brought you in today?  Why are you here today?  What seems to be the problem?

Registration Questions

  • What is your name?

  • What is your date of birth?

  • What is your address?

  • Can I see your OHIP card or other health insurance plans?

  • What is your family doctor name?

  • Do you have any allergies?

  • Are you on any medication? [take a list or bottles of medication(s)]

  • Who is your emergency contact and what is their phone number?

Doctor

  • What brings you in today?

  • What are your symptoms?

  • How long has this been going on?

  • From 1 to 10 what is your pain level right now?

     0   no pain 

1 - 3   mild pain

4 - 6   moderate pain

7 - 9   severe pain

   10   worst pain

Visiting Someone in the Hospital​

​

  • Can you tell me what room patient (Name) is in?

  • What are your visiting hours?

  • Can I visit someone in ICU (Intensive Care Unit)?

  • Can I take food for the patient?

  • Can I stay overnight with a patient?

Checklist

  • Did you explain what information you needed?

  • Did you ask all the questions?

  • Did you repeat back all the important information?  (date - time - hospital address - floor - room number)

  • Did you thank the other person for their assistance?
