  •  

    • Personal information Social Insurance Number - Address - Date of Birth - Marital Status - Dependents 

    • Charitable donations

    • Child care payments

    • Home office expenses

    • Interest on loans paid to earn investment income

    • Medical expenses

    • Moving expenses for which you were not reimbursed

    • Pension income

    • Prior year’s notice of assessment

    • Prior year’s tax return

    • RRSP paid during the year up to the end of February

    • Spousal support payment received or made

    • Student loan interest

    • Tuition fees

    • T3 - trust and dividend income

    • T4 - employment income

    • T4A - old age security

    • T5 - investment income

    • T5007 - social assistance
